Сonsent to the processing of personal data

I hereby, as a subject of personal data, in accordance with Article 9 of the Federal Law of 27.07.2006 No. 152-FZ "On Personal Data" freely, by my own will and in my own interest, give my consent to the Federal State Autonomous Educational Institution of Higher Education "Far Eastern Federal University" (FEFU), registered at the address Vladivostok, Russky Island, Ajax village, 10, to process my personal data, in accordance with Article 9 of the Federal Law of July 27, 2006 No. 152-FZ "On Personal Data", namely : last name, first name, patronymic, contact phone number, e-mail, for the purpose of booking a room in the FEFU hotel complex.

I agree to FEFU to perform actions (operations) with my personal data, as defined by Article 3 of Federal Law No. 152-FZ of July 27, 2006 "On Personal Data", namely: collection, recording, systematization, accumulation, storage, clarification (updating, changing ), extraction, use, depersonalization, blocking, deletion, destruction. This list of actions (operations) with my personal data is exhaustive and cannot be changed without my written consent. The transfer of my personal data to third parties is possible only on the basis of the current federal law or with my special written consent.

I undertake to promptly, within a period not exceeding 5 (five) working days, inform FEFU about changes in my personal data.

The processing of my personal data can be carried out both with the use of automation tools and without their use (on paper).

I give this consent for the period necessary to achieve the purposes of processing personal data, as well as for the period during which my personal data is subject to storage in accordance with the legislation of the Russian Federation.

The revocation of this consent can be carried out by me in writing or in the form of an electronic document certified by an enhanced qualified electronic signature. I take full responsibility for the adverse consequences of withdrawal of consent.

I confirm that I am familiar with the provisions of the Federal Law of 27.07.2006 No. 152-FZ "On Personal Data", including the procedure for withdrawing consent to the processing of personal data; the rights and obligations in the field of personal data protection have been explained to me.

I confirm that I have been informed that if I withdraw my consent to the processing of personal data, FEFU has the right to continue processing my personal data without my consent on the basis of Part 2 of Article 9 of Federal Law No. 152-FZ dated 27.07.2006 "On Personal data ", namely if there are grounds specified in clauses 2-11 of part 1 of article 6, part 2 of article 10 and part 2 of article 11 of this Federal Law.
