Analysis of the reasons for the failure of the ZGODA-SULZER 6ZA 40S marine diesel engine in operation

Abstract. The object of the article is the maintenance and repair of the power plant of a fishing vessel, the subject is the circumstances of the failure of the main engine. The purpose of the work is to establish the prerequisites and reasons for the failure of the Zgoda-Sulzer 6ZA 40S marine diesel engine. The analysis of the established facts that caused the accident of the main power plant (MPP) can serve as a reliable foundation for the development of preventive measures to improve the safety of navigation, by means of a posteriori analysis of information about an incident at sea. The data sources were information from departmental information resources, expert opinions on the investigation of an accident, reports from crew members, expert opinions, scientific and technical information on the analysis of the operational part of the life cycle of ship power plants (SPP), as well as the results of personal research in this area. A set of dysfunctions of the shore personnel and the crew of the fishing vessel that caused the accident has been established. The term «dysfunction», within the framework of this work, is equivalent to the concept of "failure to perform a prescribed function or its incorrect implementation», synonymous with the English concepts of «dysfunction», «malfunction". Improving the safety culture and the quality of the «human factor» contribute to minimizing incorrect maintenance of ship equipment. The presence and development of hidden operational defects, not detected by the ship's technical service until the moment of critical changes, was the main reason for the failure of the main engine.
